Diabetes Mellitus and Chronic Kidney Disease: Pathogenesis, Clinical Progression, and Therapeutic Strategies

Dear Colleagues,

Diabetes mellitus remains one of the leading causes of chronic kidney disease (CKD) worldwide and a major contributor to end-stage renal disease, cardiovascular complications, and premature mortality. The complex interplay between metabolic dysregulation, hemodynamic changes, inflammation, and fibrosis drives the development and progression of diabetic kidney disease (DKD), posing significant challenges for early detection and long-term management.

Despite advances in glycemic control strategies and renoprotective therapies, many patients continue to experience progressive renal decline. Increasing evidence highlights the importance of early risk stratification, biomarker identification, and targeted therapeutic interventions to slow disease progression and reduce associated cardiovascular risks.

Recent breakthroughs—including SGLT2 inhibitors, GLP-1 receptor agonists, mineralocorticoid receptor antagonists, and novel anti-inflammatory and antifibrotic agents—have transformed the therapeutic landscape. However, optimizing individualized treatment strategies and improving long-term renal and cardiovascular outcomes remain critical priorities.

This Special Issue aims to provide a comprehensive overview of the mechanisms underlying diabetic kidney disease and to explore current and emerging approaches to prevention, diagnosis, and management. We welcome high-quality submissions addressing, but not limited to:

• Molecular and cellular mechanisms of diabetic kidney disease 
• Pathophysiology of hyperglycemia-induced renal injury 
• Biomarkers for early detection and risk prediction 
• Clinical progression and staging of CKD in diabetes 
• Cardiovascular–renal interactions in diabetic patients 
• Pharmacological advances and reno protective therapies 
• Lifestyle interventions and multidisciplinary care models 
• Long-term outcomes and health system approaches 

Through this Special Issue, we aim to bring together endocrinologists, nephrologists, internists, and clinical researchers to advance understanding and improve patient-centered outcomes in diabetes-related kidney disease.

We invite original research articles, systematic reviews, meta-analyses, clinical studies, and comprehensive review articles.

Submitted manuscripts must be original, not previously published, and not under consideration elsewhere. Authors should prepare manuscripts according to the journal’s Instructions for Authors.
