Background: Rice puff is a popular cereal snack, especially in Asian countries, but its high Glycaemic index (GI) raises concern for diabetes management.
Objective: This review evaluates the nutritional profile, Glycaemic response, and pharmacological role of rice puff in the context of diabetes.
Methods: Literature from PubMed, Scopus, and Web of Science was reviewed, focusing on rice puff, Glycaemic index, antioxidants, and diabetes-related outcomes.
Results: Rice puff is calorie-light but nutrient-poor, with low protein, fiber, and antioxidant content. The puffing process reduces micronutrients and enhances starch digestibility, leading to rapid postprandial hyperglycaemia. Epidemiological data link refined rice intake with increased type 2 diabetes risk. Preclinical studies suggest potential antioxidant and lipid-modulating effects when rice puffs are fortified with Fibers, polyphenols, or other bioactive compounds. However, robust human clinical evidence remains scarce, and variability in puffing techniques further complicates its nutritional and metabolic evaluation.

Puffed rice (murmura) is a lightweight, easily digestible cereal with a high Glycaemic index (GI ≈ 80–90), promoting rapid postprandial glucose elevations [1,2]. Such refined rice products increase type 2 diabetes risk by raising insulin demand, while low protein and fibre limit satiety, potentially worsening Glycaemic control [3]. Despite its cultural popularity in South Asia, puffed rice poses metabolic challenges for individuals with diabetes, necessitating evaluation of its nutritional and glycaemic impact [3,4].
2.1 Macronutrients
Puffed rice is primarily composed of carbohydrates, accounting for nearly 75–80% of its dry weight, while protein content remains low at 6–7% and fat content is minimal (<1%) [5,6]. Its low fat and protein fractions contribute to reduced satiety and rapid digestibility, thereby promoting sharp postprandial Glycaemic responses [7,8]. The absence of dietary fibre further aggravates its Glycaemic potential compared with whole-grain cereals [9].
2.2 Micronutrients
The puffing process significantly reduces the micronutrient content of rice, particularly water-soluble vitamins such as thiamine and niacin [10]. Although small amounts of iron, folate, and B-vitamins may remain, the overall nutrient density is poor [11,12]. In countries like India, puffed rice is often consumed without fortification, which limits its role as a source of essential micronutrients [13]. Fortification practices, however, may improve the availability of iron and certain vitamins [14].
2.3 Glycaemic Index (GI) and Glycaemic Load (GL)
Puffed rice exhibits a high Glycaemic index (≈80–90), placing it among the rapidly digestible carbohydrate sources [5,15]. The lack of fibre and protein results in steep rises in postprandial glucose and insulin demand, which are detrimental for individuals with diabetes mellitus [7,16]. While Glycaemic load (GL) depends on portion size, even moderate servings of puffed rice can elicit a high Glycaemic response due to its rapid digestibility and low nutrient density [8,17].
Diabetes mellitus is a chronic metabolic disorder primarily characterised by persistent hyperglycaemia, resulting from a combination of insulin resistance and pancreatic β-cell dysfunction. The interplay of these defects contributes to progressive deterioration of glucose homeostasis.
3.1 Insulin Resistance and β-cell Dysfunction
In type 2 diabetes mellitus (T2DM), peripheral tissues such as skeletal muscle, liver, and adipose tissue exhibit reduced sensitivity to insulin, leading to impaired glucose uptake and increased hepatic gluconeogenesis [18]. Initially, pancreatic β-cells compensate by secreting higher amounts of insulin; however, chronic hyperinsulinemia eventually results in β-cell exhaustion and apoptosis [19,20]. Genetic predisposition, lipotoxicity, and glucotoxicity further accelerate β-cell failure [21,22].

3.2 Oxidative Stress and Inflammation
Persistent hyperglycaemia generates excessive reactive oxygen species (ROS), causing oxidative stress, which impairs insulin signalling pathways and damages β-cell function [23,24]. Simultaneously, adipose tissue in obese and insulin-resistant individuals releases pro-inflammatory cytokines such as TNF-α and IL-6, which interfere with insulin receptor signalling and promote systemic inflammation [25]. This chronic low-grade inflammation contributes to both insulin resistance and vascular complications associated with diabetes [26,27]. Thus, the pathophysiology of T2DM involves a vicious cycle of insulin resistance, β-cell dysfunction, oxidative stress, and inflammation, forming the mechanistic basis for therapeutic interventions in pharmacology.

4.1 Glycaemic Index and Postprandial Glucose Response
Puffed rice has a very high Glycaemic index (GI), reflecting rapid starch gelatinisation and porous structure that accelerates amylolysis, leading to steep postprandial glucose excursions in people with impaired glucose tolerance and diabetes [28,29]. High-GI meals increase early postprandial insulin demand, aggravate glucotoxic stress on β-cells, and worsen short-term Glycaemic variability—factors linked to microvascular risk [30,31].
4.2 Modulation of Insulin Sensitivity
Repeated exposure to high-GI loads contributes to hyperinsulinemia and peripheral insulin resistance via down-regulation of insulin signalling in muscle and liver; lowering dietary GI/GL improves insulin sensitivity in trials and cohorts [32]. Pairing puffed rice with protein, viscous fibre, or organic acids (e.g., legumes, vegetables, yoghurt, lemon/vinegar) slows gastric emptying and starch digestion, attenuating glycemia and insulin demand [33-34].

4.3 Effects on Lipid Metabolism
Although puffed rice is low in fat, chronically high Glycaemic exposure promotes hepatic de novo lipogenesis, raising triglycerides and reducing HDL-C—patterns frequently observed in insulin-resistant states [35,36]. Diets emphasising low-GI carbohydrates and higher fiber improve triglycerides and non-HDL-C in type 2 diabetes compared with high-GI patterns [37,38].

4.4 Antioxidant and Anti-inflammatory Potential
The puffing process reduces phenolics and antioxidant capacity relative to whole or minimally processed rice, offering limited protection against postprandial oxidative stress [39]. High-GI/GL patterns are associated with higher oxidative and inflammatory markers (e.g., cytokines, CRP) that impair insulin signalling; conversely, lowering GI or adding antioxidant- and fibre-rich accompaniments can mitigate these responses [40].
4.5 Additional Diabetes-Relevant Considerations
Starch structure & resistant starch: Puffed rice is dominated by rapidly digestible starch with minimal resistant starch; higher amylose or parboiled/retrograded preparations yield lower GI and improved postprandial profiles compared with puffed forms [41,42]. Satiety & energy intake: High-GI, low-protein/low-fiber foods like puffed rice provide poor satiety, predisposing to larger subsequent intakes and Glycaemic loads—undermining dietary therapy in T2D [43,44].
5.1 Low-Calorie Snack
Puffed rice is lightweight and low in energy, helping control calorie intake and Glycaemic load in diabetic diets [45,46].
5.2 Comparison with Other Cereals
Compared to extruded corn, wheat, or fried snacks, puffed rice is lower in fat and energy but has higher GI due to starch digestibility; it lacks fibre and bioactives unless fortified [47,48].
5.3 Role in Weight Management
Low-energy-density and high-volume nature promotes satiety; pairing with protein, legumes, or vegetables can aid calorie reduction, improve insulin sensitivity, and reduce diabetes risk [49,50].
6.1 Preclinical Evidence
Puffing increases rapidly digestible starch and reduces resistant starch, raising Glycaemic response [51]. Processed rice also has lower antioxidants, limiting protection against oxidative stress [52].
6.2 Acute Human Trials
Puffed rice causes higher postprandial glucose and insulin than parboiled or high-amylose rice [53,54]. Adding fibre, protein, or acids can reduce this effect [55,56].
6.3 Long-Term Evidence
Low-GI diets improve glycemia and insulin sensitivity; high-GI intake, including puffed rice, increases diabetes risk [57,58].
6.4 Fortification/Functional Modifications
Adding micronutrients, fibre, or protein can enhance nutrition and lower effective GI, but intrinsic high GI remains unless properly reformulated [59,60].
7.1 In-vitro and Preclinical Mechanisms (matrix, starch, and antioxidant loss)
Puffing expands rice kernels, increases porosity, and promotes starch gelatinisation, shifting starch toward rapidly digestible fractions with lower resistant starch—changes that predict higher Glycaemic responses [61]. Processing also diminishes phenolics and antioxidant capacity relative to minimally processed rice, reducing intrinsic defence against postprandial oxidative stress relevant to β-cell dysfunction [62].
7.2 Acute Human Feeding Studies
Processed rice formats, including puffed rice, cause higher postprandial glucose and insulin compared to high-amylose or parboiled rice [63,64]. Substituting high-amylose or retrograded rice reduces glucose and insulin excursions [65,66]. Meal composition, such as added protein, viscous fibre, or organic acids, further attenuates glycemic and insulinemic responses [67,68].
7.3 Longer-term Clinical Evidence
Low-GI/GL diets improve glycemic control, insulin sensitivity, and lower HbA1c and triglycerides in type 2 diabetes [69,70]. High white-rice intake increases diabetes risk, emphasising lower-GI rice and portion control [71]. Resistant-starch–rich interventions from high-amylose sources enhance postprandial metabolism and insulin sensitivity over weeks to months [72,73].
7.4 Vascular and Oxidative Stress Endpoints (mechanistic clinical signals)
Clinical physiology studies link Glycaemic variability and oscillating hyperglycaemia to endothelial dysfunction and oxidative stress—mechanisms aggravated by rapidly digestible carbohydrates—supporting dietary minimisation of high-GI staples such as puffed rice in diabetes. Lower-GI diets or meals enriched with viscous fibre and legumes reduce inflammatory and oxidative markers in type 2 diabetes, aligning with pharmacologic goals [74].
8.1 Safety Concerns
Puffed rice is widely consumed and generally recognised as safe. However, safety issues may arise due to contamination, nutrient loss, or high Glycaemic properties. During puffing, rice loses dietary fibre, vitamins, and antioxidants, leaving a carbohydrate-rich food with limited protective nutrients [75]. Moreover, puffing at high temperatures can generate advanced glycation end-products (AGEs) and lipid peroxidation products, which may worsen oxidative stress in individuals with diabetes [76]. Food safety concerns also include possible arsenic accumulation in rice-based products, which can pose chronic health risks [77].
8.2 Limitations in Diabetes Management
The primary limitation of puffed rice in diabetes is its high Glycaemic index (GI) and Glycaemic load (GL), which induce sharp postprandial hyperglycaemia and hyperinsulinemia [78]. Frequent consumption may contribute to Glycaemic variability, insulin resistance, and progression of diabetic complications [79]. Furthermore, unlike intact or high-amylose rice, puffed rice lacks resistant starch, resulting in minimal prebiotic or gut-metabolic benefits [80]. Nutrient density is also limited compared to whole-grain or fortified rice products, reducing its functional value for diabetes prevention or management [81].
8.3 Challenges and Future Perspectives
The challenge in utilising puffed rice for diabetes lies in balancing cultural acceptance with metabolic safety. Puffed rice is deeply embedded in traditional diets, but its metabolic drawbacks require innovative approaches. Possible strategies include fortification with protein, soluble fibre, or bioactives to lower GI and enhance antioxidant potential [82]. Reformulation studies suggest that blending puffed rice with legume flours, resistant starch, or polyphenols may mitigate Glycaemic spikes, though clinical validation remains limited [83]. Another challenge is the lack of large-scale randomised controlled trials directly evaluating puffed rice in diabetic patients; most available evidence is extrapolated from broader rice-GI research [84]. Thus, while puffed rice is culturally important and safe in moderation, its nutritional limitations, high Glycaemic impact, and processing-related risks demand cautious dietary recommendations in diabetes care.
9.1 Functional Food Development
Functional modifications—adding soluble fibre, protein, polyphenols, or resistant starch—can lower puffed rice’s Glycaemic impact and improve satiety. Blending with legumes/millets or processing via extrusion/enzymes may create safer, low-GI alternatives [85,86].
9.2 Role in Combination Therapy
Puffed rice can deliver bioactives (polyphenols, curcumin, rice bran) to reduce oxidative stress and inflammation. Combining with α-glucosidase inhibitors may lessen postprandial glucose, potentially supporting diabetes therapy [87,88].
9.3 Potential for Nutraceutical Formulation
Puffed rice may also function as a delivery vehicle in nutraceutical design. Micronutrient fortification (iron, B-vitamins) has already been demonstrated in puffed cereals, and expanding this approach to diabetes-specific bioactives (chromium, magnesium, tocotrienols, or phytosterols) could enhance metabolic regulation [89]. Research into encapsulation technologies shows that puffed cereal matrices can carry probiotics, prebiotics, and polyphenols with good stability, suggesting a pathway for creating rice puff–based nutraceuticals tailored to Glycaemic control [90]. Further clinical trials and regulatory evaluations will be essential to establish efficacy, safety, and scalability for therapeutic application [91].
The evidence reviewed indicates that while rice puff is a culturally important and widely consumed cereal product, its nutritional and metabolic profile presents substantial challenges for diabetes management. The high Glycaemic index and Glycaemic load of puffed rice consistently translate into rapid postprandial hyperglycaemia and hyperinsulinemia, mechanistically linked to β-cell stress and progressive insulin resistance [92]. This aligns with large-scale epidemiological studies showing positive associations between high intake of refined or processed rice and incident type 2 diabetes across multiple populations [93]. Nevertheless, rice puff’s role as a low-calorie, convenient snack provides opportunities for reformulation and functional food development. Several experimental approaches, including fortification with soluble fibres, proteins, or polyphenolic compounds, have demonstrated potential to attenuate postprandial responses [94]. Moreover, incorporation of puffed rice into composite foods with legumes or millets may help reduce Glycaemic impact while maintaining cultural acceptability [95]. However, clinical trials directly testing modified puffed rice formulations in diabetic or prediabetic cohorts remain scarce. Oxidative stress and inflammation are central to the pathophysiology of diabetes, and processing-induced nutrient losses may limit puffed rice’s protective capacity. The reduction in phenolics and antioxidant activity observed in puffed products suggests diminished defences against hyperglycaemia-induced oxidative damage [96]. While fortification strategies—such as inclusion of rice bran extracts or tocotrienols—have shown lipid-lowering and antioxidative effects in other rice-based foods, direct application to puffed rice has yet to be fully validated [97]. From a pharmacological standpoint, puffed rice could serve as a dietary carrier matrix for nutraceuticals or bioactives. Combination with α-glucosidase inhibitors or antioxidants offers theoretical synergy with pharmacotherapy, but human evidence is currently indirect and extrapolated from broader rice studies [98]. This represents both a limitation and an opportunity for future translational research. Critical challenges include the lack of randomised controlled trials assessing puffed rice specifically in diabetes, variability in puffing techniques influencing nutritional outcomes, and concerns about potential contaminants such as arsenic or advanced glycation end-products generated during processing [99,100]. Addressing these issues requires interdisciplinary work bridging food technology, nutrition, and clinical pharmacology. In summary, the discussion highlights a dual perspective: puffed rice, in its conventional form, poses significant risks for Glycaemic control in diabetes; yet, with targeted modifications, it holds promise as a culturally acceptable vehicle for functional and therapeutic innovations. The task ahead lies in moving from observational and mechanistic evidence to well-designed clinical studies that validate safe and effective roles for puffed rice in diabetes management.
Rice puff has a dual role in diabetes: its high Glycaemic index, low protein, and reduced antioxidants worsen glycaemic control and oxidative stress, while its low calorie, cultural acceptance, and versatility allow functional and therapeutic use. Fortification, co-processing with fibers/legumes, and bioactive incorporation may improve insulin sensitivity and lipid metabolism. Preclinical results are promising, but human trials are needed. Traditional rice puff is not recommended for diabetes, but modified versions could serve as functional foods or adjunct therapy, pending safety and long-term studies.

Dear Mayra Duenas, Editorial Coordinator of the journal IJCCR, I write here a little on my experience as an author submitting to the International Journal of Clinical Case Reports and Reviews (IJCCR). This was my first submission to IJCCR and my manuscript was inherently an outsider’s effort. It attempted to broadly identify and then make some sense of life’s under-appreciated mysteries. I initially had responded to a request for possible submissions. I then contacted IJCCR with a tentative topic for a manuscript. They quickly got back with an approval for the submission, but with a particular requirement that it be medically relevant. I then put together a manuscript and submitted it. After the usual back-and-forth over forms and formality, the manuscript was sent off for reviews. Within 2 weeks I got back 4 reviews which were both helpful and also surprising. Surprising in that the topic was somewhat foreign to medical literature. My subsequent updates in response to the reviewer comments went smoothly and in short order I had a series of proofs to evaluate. All in all, the whole publication process seemed outstanding.
