Essential tremor (ET) is a common hyperkinetic movement disorder that can considerably impair daily functioning and quality of life. The VILIM ball is a handheld, CE-marked vibrotactile medical device marketed to provide temporary reduction of hand tremor associated with ET. We systematically reviewed the publicly available clinical evidence evaluating its benefit in ET, with particular attention to study design, outcome measures, methodological quality, probable conflicts of interest, and the feasibility of quantitative synthesis. To identify relevant studies, we conducted a structured literature search in PubMed and Scopus for articles published through March 2024. The search terms included "essential tremor," "vibrotactile device," "VILIM ball," and "hand tremor." Inclusion criteria were peer-reviewed clinical studies, trials, or surveys evaluating the VILIM ball in patients with essential tremor. Exclusion criteria consisted of non-original studies, studies not involving the VILIM ball, and studies focused on other conditions or interventions. Reference lists of identified articles and trial registries were also screened to guarantee comprehensive coverage. The only studies that had been published were a sham-controlled trial and a cross-sectional survey without a control group, together with an ongoing registered trial and a number of unpublished findings reported by the manufacturer. The sham-controlled trial demonstrated a statistically significant, device-specific reduction in tremor power in approximately three-quarters of participants, whereas sham stimulation produced no comparable effect. However, the body of evidence remains limited due to the small number of studies, the use of diverse outcome measures, and the considerable overlap between study authors and manufacturers. It is important to note that when the claim made by the manufacturer—asserting that corroborative evidence from the ULTRE trial could be attributed to the VILIM ball—was verified, it was found that the trial in fact had evaluated a different and competing device, thus stressing the need for primary-source verification when assessing new medical-device evidence.Given the paucity of controlled studies and methodological heterogeneity, quantitative meta-analysis is not currently feasible, and the evidence may provide a short-term reduction in hand tremor for some individuals with ET, particularly those with mild to moderate symptoms who are either refractory to, intolerant of, or inadequately controlled by standard pharmacologic treatments. The device may be reasonably considered for patients seeking non-invasive adjunctive options, although the available evidence is insufficient to confidently establish its clinical efficacy. Given the current state of evidence, clinicians may consider counseling patients that the VILIM ball remains an experimental adjunctive treatment. Clinicians are advised to engage in shared decision-making by outlining the limited quality of the available evidence, the absence of long-term data, and the need for additional research, while transparently communicating both the possibility for benefit and the remaining uncertainties.Looking forward, there is a clear need for additional research to address existing gaps and improve the quality of the evidence base. Specifically, multicenter randomized controlled trials (RCTs) with adequate power and independent investigators ought to be prioritized. These studies should utilize validated, blinded, clinician-rated tremor severity scales as primary outcomes, assess functional and quality-of-life outcomes, and include longer-term follow-up to evaluate durability and safety. Additionally, the inclusion of consistent protocols and harmonized outcome measures across studies would facilitate comparison with other devices and interventions. Such research is vital to clarify the true effectiveness, clinical relevance, and long-term impact of the VILIM ball in essential tremor.
Essential tremor (ET) is among the most common movement disorders in adults, with prevalence estimates varying by age, ethnicity, and diagnostic criteria [11,12], producing a postural and kinetic (action) tremor that most often affects the hands and can substantially impair activities of daily living. First-line pharmacologic therapy (propranolol, primidone, gabapentin, and topiramate) is effective in only a subset of patients, and although several agents have been explored for symptomatic relief, treatment response is often incomplete or inconsistent [3], motivating interest in non-pharmacologic adjuncts. For patients with medication-refractory tremor, surgical options such as thalamotomy exist but carry procedural risk and are not appropriate for all patients [7]. These limitations have motivated interest in noninvasive alternatives, including peripheral nerve stimulation and other peripheral or mechanical approaches [10.18], as well as non-invasive brain stimulation techniques [20].
Vibration-based approaches to tremor suppression draw on a broader literature examining how mechanical stimulation affects motor output, including work on stochastic resonance and noise-based modulation of motor performance [21,22] and on wrist tendon vibration's effects on upper-limb movement in stroke rehabilitation [6]. Related device-based approaches to tremor suppression have also been explored, including self-balancing mechanical devices [5] and powered exoskeletal systems [8].
The VILIM ball, manufactured by Vilimed (Lithuania), is a handheld, CE-marked (class 2a) vibrotactile device designed for repeated daily use. The recommended usage is three sessions per day, with each session comprising 5–10 minutes of device application to the affected hand. This regimen is intended to produce a temporary reduction in hand tremor by delivering mechanical vibration at 8–18 Hz with an amplitude of 0–2 mm, for repeated daily use.
Rationale and Proposed Mechanism
The way in which local vibrotactile stimulation might lead to a reduction in tremor amplitude is based on the principles of sensory gating and the modulation of somatosensory feedback: it is thought that applying high-frequency mechanical vibration to the hand modifies the afferent proprioceptive signaling and the central oscillatory drive affecting the muscles responsible for tremor generation, without directly acting on the central oscillator circuits (the cerebello-thalamo-cortical loop) involved in the pathophysiology of essential tremor. This mechanism differs from pharmacological methods, which have a central action or affect peripheral beta-adrenergic transmission, and from surgical or focused ultrasound methods, which directly lesion or modulate the ventral intermediate nucleus of the thalamus. Since this is a peripheral, non-invasive, and sensory-based intervention, the size of its plausible effect is expected to be small, and it is likely to be influenced by the placebo response, which is why a sham-controlled evaluation is especially important for this type of device.
Sham-Controlled Trial
The sole study that included a sham control recruited 17 patients diagnosed with essential tremor and employed a prospective, single-center, single-arm, pragmatic approach, with an extended sham-controlled safety phase that added 15 more patients (for a total of 32 patients exposed in both arms). Of the 17 patients, 13 (76%) showed a statistically significant decrease in the power of their dominant-hand tremor after using the device, with an average reduction of about 56% to 60% (with a significance level of α=0.0352); the sham device caused no measurable reduction in tremor [1]. The conclusion stated by the study itself was reasonably cautious in that it said the device reduces involuntary movement in some patients and is safe to use [1].
Cross-Sectional Real-World Survey
A separate, non-randomized cross-sectional study conducted telephone interviews with 51 long-term users of the device (31 with ET and 20 with parkinsonian tremor). Of the 51, 48 (94%) reported improvement in tremor symptoms and 49 (96%) reported improvement in function, with benefits lasting on average 90.79 minutes (SD 68.83) per session over a mean period of 7.63 months (SD 5.41) [17]. Two patients reported a lack of efficacy of the device. The main limitation of this study is that it used an unvalidated, unblinded patient-reported outcome (a telephone questionnaire) which was given to existing device owners, thus introducing a considerable amount of selection and response bias. It is also worth noting that this investigation was conducted by scientists affiliated with MB "Fidens," the company that invented the VILIM ball, representing a further potential conflict of interest beyond the sham-controlled trial discussed above [17].
Ongoing Registered Trial
A new controlled trial which has now been registered (the trial compares the device with a control group that receives alternative therapy and involves independent clinician assessment based on video recordings) is specifically intended to address the gaps in the previous literature — the registration document points out that the earlier study had no training phase, no control group, and had not assessed the effect on patients' daily activities. At the time of this review, the results had not yet been available.
Unpublished and Manufacturer-Reported Results
Other results mentioned by the manufacturer as supporting evidence—such as a voluntary customer survey involving 143 people and two further small studies in patients with Parkinson's disease—have not been made available as peer-reviewed, independently accessible publications and should therefore be regarded as preliminary marketing claims rather than as clinical evidence until they have been published in a peer-reviewed manner.
Independent and Non-Manufacturer Perspectives
The results of an independent, structured panel review (which included six reviewers over a four-week trial period) were mixed in that most of the reviewers did not have any trouble using the device, one of them noted a 40%-50% subjective improvement in tremor—especially when going to bed—although the same reviewer also observed that the device might sometimes make the shaking worse and that it was tiring to hold it steadily; another reviewer found it difficult to fit the recommended three-times-daily routine within a busy and active way of life. Likewise, discussion within the patient community shows mixed outcomes, with some users reporting a modest benefit and others reporting no benefit at all. A recent independent, peer-reviewed review of peripheral and mechanical devices for essential tremor similarly concluded that Vilim ball therapy might provide temporary relief from symptoms for some individuals, but that more robust, large-scale research is needed to conclusively establish its efficacy, optimal usage parameters (frequency and duration of use), and long-term benefits [2]. The same review noted two practical limitations independent of efficacy: the hand cannot be used for any other task while holding the device, and it remains unclear whether any tremor reduction persists after use has stopped [2].
Reported Adverse Effects and Tolerability Issues
The adverse effects and tolerability concerns mentioned in the existing literature are usually mild, but patients should be given consideration of these points. They consist of a temporary worsening of tremor while using the device or shortly after use, hand fatigue or discomfort from holding the device for several sessions each day, and practical problems associated with sticking to the recommended treatment schedule. Some users have also found it difficult to fit the device into their everyday routines, and a small number reported obtaining no benefit or stopped using it because it was ineffective. However, to date, there have been no serious adverse events reported in the published literature, although systematic safety reporting remains limited and further studies may yield additional information.
The currently available best-quality evidence—a small sham-controlled trial involving 32 participants in total—shows a statistically significant reduction in tremor power in the majority of ET patients tested, whereas the sham condition produced no similar effect [1]. This does represent a real, although still preliminary, positive indication. Nevertheless, the overall body of evidence is based on a single controlled trial, one uncontrolled survey using an unvalidated outcome measure, and a number of unpublished results which cannot be independently verified. To date, no study has employed a validated, blinded, clinician-rated tremor severity scale as its main outcome measure in a patient group specifically affected by ET, and all of the published, peer-reviewed evidence so far has involved authors who are also associated with the manufacturer, a fact which raises a conflict of interest and calls for attentive interpretation [1,17].
It is not currently possible to carry out a formal pooled meta-analysis since there are fewer than two suitable sham-controlled trials available and the outcome measures are not standardized among the existing studies (some studies use the technical tremor power spectrum, others rely on an unvalidated patient questionnaire, and still others employ functional measures from the physiotherapy-adjunct literature), which means that a meaningful pooled effect size cannot be calculated. The review is therefore presented as a narrative synthesis and critical appraisal. Among the research priorities for the future should be an adequately powered, multicenter, sham-controlled randomized trial conducted by independent investigators, using a validated clinician-rated tremor scale as the main outcome measure, and the adoption of standardized functional outcome measures to enable cross-device comparisons within the wider category of vibrotactile therapy. When compared with other commercially available vibrotactile or assistive devices, such as wrist-worn wearables and orthotic tremor-suppression aids, the clinical evidence for the VILIM ball remains very limited in both quantity and independent validation. Although other devices, such as the ULTRE wrist-worn stimulator and various adaptive utensils, have been the subject of larger or more diverse controlled studies [14], the data on the VILIM ball are primarily found in manufacturer-associated publications and do not include independent, blinded assessment as a primary outcome. This situation is important for clinicians when evaluating the range of devices available for managing essential tremor. The currently registered trial, which appears to be the most methodologically rigorous assessment of the device to date, has been completed.
Vibration therapy for tremor is more broadly characterized in the wider tremor-device literature as having inconsistent evidence for efficacy, a framing echoed by a recent independent review of tremor-suppression orthoses that lists vibration therapy among therapeutic approaches for which "some research reports no efficacy" [13].
It is also worth situating vibrotactile devices such as the VILIM ball within the wider literature on vibration exposure and safety. Occupational studies of hand-arm vibration have documented associations with vascular and neurological injury [15], musculoskeletal disorders [4], and hand-arm vibration syndrome sufficient to cause work disability [19]. While these findings arise from chronic, high-intensity occupational exposure rather than the brief, low-amplitude therapeutic sessions used with the VILIM ball, they highlight the importance of continued safety monitoring as vibrotactile devices see wider and more prolonged clinical use.
The current evidence offers a preliminary yet real indication that the VILIM ball causes a device-specific decrease in tremor power in patients with essential tremor, an effect that can be distinguished from a sham effect in the only trial carried out so far [1]. Nevertheless, the body of evidence remains in its early stages and is limited by the small number of published studies, the generally small sample sizes, the lack of independent research, the insufficient use of blinded and validated clinician-assessed outcome measures, and a high degree of variability in study designs and endpoints. Because of these limitations, it is not possible to draw definitive conclusions about the device's true clinical impact, and it is difficult to generalize the findings to broader patient groups. Consequently, both clinicians and patients should regard the existing estimates of efficacy (ranging from 56% to 94% depending on the study design and outcome measure) as a broad, poorly combined range that reflects methodological discrepancies rather than showing a single reliable effect size. In practice, these shortcomings underscore the need for clear, open discussions with patients about the device's experimental nature, the possibility that it may benefit some but not all, and the fact that high-quality research must continue before the device can be incorporated into routine clinical practice. It is essential that more rigorous, independently conducted trials be carried out to establish the therapeutic value of the VILIM ball for essential tremor.
As device-based and peripheral stimulation approaches to ET continue to diversify [10,18], rigorous, independent evaluation—both of efficacy and of the safety profile established in analogous vibration-exposure contexts [15]—will be essential to determine the VILIM ball's appropriate place among these emerging non-pharmacological options.

Dear Maria Emerson, Editorial Coordinator of International Journal of Clinical Case Reports and Reviews, What distinguishes International Journal of Clinical Case Report and Review is not only the scientific rigor of its publications, but the intellectual climate in which research is evaluated. The submission process is refreshingly free of unnecessary formal barriers and bureaucratic rituals that often complicate academic publishing without adding real value. The peer-review system is demanding yet constructive, guided by genuine scientific dialogue rather than hierarchical or authoritarian attitudes. Reviewers act as collaborators in improving the manuscript, not as gatekeepers imposing arbitrary standards. This journal offers a rare balance: high methodological standards combined with a respectful, transparent, and supportive editorial approach. In an era where publishing can feel more burdensome than research itself, this platform restores the original purpose of peer review — to refine ideas, not to obstruct them Prof. Perlat Kapisyzi, FCCP PULMONOLOGIST AND THORACIC IMAGING.

Dear Mayra Duenas, Editorial Coordinator of the journal IJCCR, I write here a little on my experience as an author submitting to the International Journal of Clinical Case Reports and Reviews (IJCCR). This was my first submission to IJCCR and my manuscript was inherently an outsider’s effort. It attempted to broadly identify and then make some sense of life’s under-appreciated mysteries. I initially had responded to a request for possible submissions. I then contacted IJCCR with a tentative topic for a manuscript. They quickly got back with an approval for the submission, but with a particular requirement that it be medically relevant. I then put together a manuscript and submitted it. After the usual back-and-forth over forms and formality, the manuscript was sent off for reviews. Within 2 weeks I got back 4 reviews which were both helpful and also surprising. Surprising in that the topic was somewhat foreign to medical literature. My subsequent updates in response to the reviewer comments went smoothly and in short order I had a series of proofs to evaluate. All in all, the whole publication process seemed outstanding. It was both helpful in terms of the paper’s content and also in terms of its efficient and friendly communications. Thank you all very much. Sincerely, Ted Christopher, Rochester, NY.
