Attention Deficit Hyperactivity Disorder (ADHD) affects an estimated 3% of adults worldwide (Fayyad et al., 2017). Current national and local prescribing guidelines recommend methylphenidate and lisdexamfetamine as first-line pharmacological treatments for adults with ADHD (JFC, 2025; NICE, 2019; SBUHB, 2022). However, the limited availability of direct comparative studies creates uncertainty regarding the optimal initial treatment choice. This critical review evaluates the evidence underpinning current prescribing recommendations and examines the relative merits of methylphenidate and lisdexamfetamine as first-line pharmacological treatments for adults with ADHD.
Attention Deficit Hyperactivity Disorder (ADHD) is increasingly recognised and diagnosed in adulthood, creating growing demand for assessment and treatment within mental health services. As Advanced Nurse Practitioners (ANPs) and other independent prescribers undertake expanding roles in the management of adult ADHD, there is a corresponding need to ensure prescribing decisions are informed by robust evidence. The Royal Pharmaceutical Society Competency Framework (2021) and the Nursing and Midwifery Council (2024) emphasise the responsibility of prescribers to critically evaluate available evidence to support safe, effective, and person-centred care.
Current national and local prescribing guidelines recommend both methylphenidate and lisdexamfetamine as first-line pharmacological treatments for adults with ADHD. However, despite their widespread use, the limited availability of robust head-to-head comparative trials (Faraone & Glatt, 2009; Gutiérrez-Casares, 2021; Newcorn et al., 2017) makes it difficult to determine whether one medication should be preferred over the other as an initial treatment option. This uncertainty presents a challenge for clinicians seeking to balance efficacy, tolerability, patient characteristics, and cost-effectiveness when initiating treatment.
This critical review examines the evidence underpinning current treatment recommendations and evaluates the relative merits of methylphenidate and lisdexamfetamine as first-line therapies for adults with ADHD. In doing so, it aims to support evidence-informed prescribing and clinical decision-making by ANPs and other Non-Medical prescribers working within adult mental health outpatient services.
Evidence Base Underpinning Current Guidelines
The NICE (2019) guideline on ADHD diagnosis and management was informed by 39 double-blind randomised controlled trials (RCTs), of which 15 included adult participants. 12 studies investigated methylphenidate, while only 3 evaluated lisdexamfetamine (NICE, 2018).
RCTs are widely regarded as the gold standard for evaluating treatment efficacy as they minimise potential bias and permit causal inference (Miller, 2000). Nevertheless, the methodological quality of the studies informing the NICE recommendations has been questioned. Faltinsen et al. (2019) argued that much of the evidence was of low certainty owing to small sample sizes and relatively short follow-up periods. Indeed, 9 of the 15 adult studies lasted 6 weeks or less, limiting conclusions regarding long-term effectiveness and safety.
Several methodological limitations further reduce confidence in the available evidence. For example, Casas et al. (2013) recruited substantially more participants into the methylphenidate treatment arm (n = 182) than the placebo arm (n = 97). Although unequal allocation ratios can still generate reliable findings (Gupta & Verma, 2013), they may also lead to over or underestimation of treatment effects (Howick & Hoffman, 2018).
Furthermore, 14 of the 16 adult studies were funded or conducted by pharmaceutical companies. Industry sponsorship has the potential to introduce publication and reporting bias, particularly where commercial interests are associated with drug licensing and financial gain (Jefferson, 2020). However, given the substantial financial costs associated with large-scale clinical trials, pharmaceutical industry involvement remains common within drug development (Mielke & Rohde, 2021). When ethical standards and rigorous methodology are maintained, such studies continue to make an important contribution to the evidence base (Gallin et al., 2018). Consequently, prescribers must critically evaluate study quality rather than dismiss findings solely based on funding sources.
Applicability of the Evidence to Adult ADHD Populations
Despite the methodological limitations, all RCTs reviewed by NICE (2018) reported 95% confidence intervals demonstrating that treatment with either methylphenidate or lisdexamfetamine produced superior improvements in ADHD symptoms and functional outcomes compared with placebo groups. These findings suggest that treatment effects are likely to be reproducible across similar populations (Elkins, 2024).
However, closer examination of participant characteristics raises important questions regarding external validity. For example, one of the lisdexamfetamine trials included only participants aged 18–26 years (Biederman et al., 2012), potentially limiting the applicability of findings to older adults with ADHD. None of the included studies enrolled adults over 65 years of age, making long-term treatment outcomes across the lifespan difficult to predict.
Representation of female participants was also inconsistent. Two studies included no women (Ginsberg et al., 2012; Goodman et al., 2017), while several failed to report participant ethnicity (Biederman et al., 2006; Retz et al., 2012). These omissions reduce confidence in generalising findings across the broader population of adults with ADHD.
Consideration of study population characteristics is a key component of critical appraisal (Nimavat et al., 2020), and caution is warranted when extrapolating findings beyond the populations originally studied (Mielke & Rohde, 2021). Nevertheless, 10 of the 12 methylphenidate studies included participant populations that were broadly representative of typical adults receiving ADHD treatment, whereas only 2 lisdexamfetamine studies demonstrated similar demographic coverage. This provides a comparatively stronger evidence base supporting methylphenidate as an initial treatment option in adult ADHD populations.
Current literature consistently demonstrates that methylphenidate has been investigated more extensively than other pharmacological treatments for adult ADHD (Cortese et al., 2018; NICE, 2018). Cochrane systematic reviews similarly concluded that both methylphenidate (Boesen et al., 2022) and lisdexamfetamine (Castells et al., 2018) effectively reduce ADHD symptoms in adults while highlighting the substantially larger body of evidence supporting methylphenidate.
Conversely, Cortese et al.'s (2018) network meta-analysis, which examined efficacy, tolerability, and adverse effects across stimulant and non-stimulant medications, reported that amphetamine-based medications, including lisdexamfetamine, may be better tolerated than methylphenidate. These findings suggest lisdexamfetamine may represent an appropriate alternative for patients who experience intolerable adverse effects with methylphenidate.
Economic considerations may also influence prescribing decisions. Under principles of prudent healthcare, treatment value should be considered alongside clinical effectiveness when considering first-line treatment options for adults with ADHD. Methylphenidate is generally associated with lower acquisition costs than lisdexamfetamine (JFC, 2025). Consequently, where clinical outcomes are comparable, methylphenidate may represent the more economical initial treatment option. However, acquisition cost alone may not fully reflect comparative treatment value.
Lisdexamfetamine is a long-acting prodrug of dexamfetamine that provides symptom control throughout much of the day following once-daily administration, potentially reducing the need for additional medication and simplifying treatment regimens (Coghill et al., 2013; Frampton, 2016). In contrast, modified-release methylphenidate preparations exhibit considerable variation in duration of action, and some patients may require supplementary immediate-release doses later in the day to maintain symptom control (NICE, 2019). Treatment complexity has been identified as an important determinant of medication adherence, and formulations requiring fewer daily doses are generally associated with improved treatment persistence (Claxton et al., 2001).
These factors suggest that the relative cost-effectiveness of methylphenidate and lisdexamfetamine may vary according to individual clinical circumstances. While methylphenidate is likely to remain the most cost-effective first-line option for many treatment-naive adults, lisdexamfetamine may offer greater value for individuals requiring extended symptom coverage or those who experience difficulties adhering to more complex dosing schedules (Cortese et al., 2018; Frampton, 2016). Therefore, prescribing decisions should be informed by both economic considerations and patient-specific treatment needs.
The available evidence indicates that methylphenidate is supported by a substantially larger and more consistent body of research than lisdexamfetamine. Although the studies informing NICE (2019) recommendations have recognised methodological limitations, the efficacy and safety of modified-release methylphenidate have been replicated across numerous clinical trials, strengthening confidence in its effectiveness.
Emerging evidence suggests that lisdexamfetamine may be better tolerated than methylphenidate, and continued research may further clarify its role within first-line treatment. This reinforces the responsibility of prescribers to remain current with evolving evidence and adapt prescribing practice accordingly.
Overall, the greater volume of supporting evidence, broader applicability across adult ADHD populations, and superior cost-effectiveness collectively provide a credible rationale for considering methylphenidate as the preferred first-line pharmacological treatment for many adults with ADHD. Individual treatment decisions should nevertheless be guided by clinical presentation, patient preference, tolerability, and emerging evidence. However, lisdexamfetamine may be preferable for some individuals because its longer duration of action can provide extended symptom control and reduce the need for additional medication later in the day. Therefore, medication selection should be guided not only by the strength of the evidence base and cost considerations, but also by individual patient needs, treatment goals, patient preference, tolerability, and emerging evidence

I initially had responded to a request for possible submissions. I then contacted IJCCR with a tentative topic for a manuscript. They quickly got back with an approval for the submission, but with a particular requirement that it be medically relevant. I then put together a manuscript and submitted it. After the usual back-and-forth over forms and formality, the manuscript was sent off for reviews. Within 2 weeks I got back 4 reviews which were both helpful and also surprising. Surprising in that the topic was somewhat foreign to medical literature.
